OSU Health Access Network

OSU Health Access Network (HAN) provides personalized case management for Oklahoma Medicaid members who need help coordinating their care. Experienced nurses and social workers partner with patients, families, and primary care providers to set health goals, manage chronic conditions, and connect individuals with community resources. HAN also offers health education, support groups, and assistance overcoming barriers such as transportation. The program empowers patients with the knowledge and tools to stay healthy and actively manage their care.

OSU HAN Case Management provides extra care and support for members of Oklahoma Medicaid who have been identified as having a special need for case management services or those who may be having difficulty managing and coordinating health care services. We also receive referrals from your primary care providers (PCP). We educate patients to help them gain the confidence, knowledge and tools needed to play an active role in managing and maintaining their health care.

Your OSU HAN Case Manager is a registered nurse, licensed practical nurse, or social worker with advanced education and experience who will work with you and your family or caregiver to look over your health care needs and help locate the resources needed to keep you healthy. We work with your PCP and any specialist you see to help you set goals and treatment plans. We teach you about your diagnosis or current illness and give you tools to live a healthier life. We help you identify what is important to you and the best way to reach your health care goals.

HAN identifies the care and information you need and connects you with appropriate resources, educational classes, and support groups. They also provide health education related to chronic disease management, preventative health and wellness, and medication management, and ensure you receive assistance with hearing, seeing or understanding this information.

Your primary care provider is a very important part of your health care team. Your case manager will work closely with your PCP, who is responsible for your health care and referrals. We will communicate with you and your PCP to make sure you get the best care possible—giving you the tools to improve your knowledge and understanding of your medical needs and how to manage them.

Keeping scheduled appointments with your provider and case manager is an important part of your health care. Repeated missed appointments get in the way of effective treatment.

Let us know of any problems you have keeping appointments and we will work with you to address those issues. If transportation to and from your clinic appointments is a barrier, let us know and we can assistance with finding a solution.

Always call and reschedule your appointment if you know you are unable to keep it.

One of our goals is to help you avoid using the emergency room when you do not have a medical emergency. However, there are times when going to the emergency room may be needed. If you are not sure you have an emergency, call your PCP.

Examples of illnesses and injuries that must be treated in the ER:

  • Loss of consciousness or unable to respond to questions
  • Pain and tightness in your chest
  • Severe pain with pregnancy
  • Stroke symptoms
  • Difficulty breathing
  • Uncontrolled bleeding
  • Sudden and severe pain
  • Poisoning/overdose
  • Head injury
  • Broken bones
  • Coughing up or vomiting blood
  • Burns with blisters
  • Severe or persistent vomiting
  • Suicidal feelings

Be sure to schedule an ER follow-up appointment with your PCP. Please let your OSU HAN Case Manager know if you go to the ER so we can help you follow up on your care.

  • Members can opt in and opt out of care management at any time that they choose.
  • Members can opt in to some or all services offered by the care management program.
  • My plan of care can change if my needs change. My plan of care can change after further review. If it changes, my care manager will talk to me about it.
  • If I need more services, I can call my care manager.
  • My care team includes my primary care provider.
